Registration Forms
The Registration Form is required for Fall, Select and Spring Registration. This form is to be filled out and turned in at the time of registration. Registration dates are normally the last weekend in July and the first weekend in August and will be posted on our website.
The Consent to Treat Form is required for Fall, Select and Spring Registration. The form is needed for EYHA to authorize necessary treatment in case a skater is injured.
The Zero Tolerance Form is rquired for Fall, Select and Spring Registration. The form is a declaration of the understanding of and agreement to abide by all EYHA rules regarding behavior and conduct.
The Jersey Agreement Form is used for Select and Travel programs only. Any player on a Select or Travel team must fill this form out and turn in to the Team Manager for their jersey.
EYHA is required by USA hockey that all skaters be registered for insurance. All players must register each season on their own for a $35.00 and submit the confirmation form with their the registration paperwork.
